Case Management – Chronic Disease

Ever wondered what is meant by the term Case Manager or Case Management? This course will explain the principles of case management and help nurses to understand the key role this plays in the success of patient care. It will explain how collaboration, assessment, planning, implementation, monitoring and evaluation play an integral part in therequirements to meet individual health needs. An effective case management system can actively manage patient care from admission to discharge. This course will introduce and explain the case management approach to the management of long-term conditions. The course will help Nurses to develop a confident approach to understanding and managing long-term conditions. You will also develop your clinical skills by understanding head-to-toe clinical examination and assessment. This course will also help you understand the importance of training and mentoring and development of competence. Competence based frameworks and clinical development plans are discussed.
